Vaccine Requirements for the State of Missouri

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start, K-12 Day Care, Head Start- 3-4 months: 1 doses. 5-6 months: 2 doses. 7-15 months: 3 doses. 16 months to K: 4 doses.

K-12: 4 doses. Last dose on or after the fourth birthday and the last dose of pediatric pertussis before the seventh birthday. Maximum needed: six doses.

Tdap, which contains pertussis vaccine, is required for students enrolled in the eighth, ninth and tenth grade who have completed the recommended childhood DTaP/DTP vaccination series and have not received a Td booster dose within the past two years. If a student received a Tdap booster the student is up-to-date. Tdap is currently licensed for one dose only; an additional dose is not needed. For 11-12 grades, a Tdap or Td booster is required 10 years after the last dose of DTaP, DTP or DT. In the event of a pertussis outbreak situation, Tdap may be given at intervals less than 10 years.

Hib Day Care, Head Start

If 1st dose given at 7-11 months - 3 doses (4 weeks between dose 1 and dose 2 with a booster dose at 12-15 months)

If 1st dose given at 12-23 months - 2 doses (8 weeks between dose 1 and dose 2)

If 1st dose given at 24-59 months - 1 dose

Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12 Day Care, Head Start, K-12- 16 months to K: 1 dose. K-12: 2 doses

K-12 - 2nd dose measles is required prior to entry

Meningococcal None/No Requirements None/No Requirements
Pneumococcal Day Care, Head Start

The number of doses a child needs to complete the series depends on the age the child begins the series.

4 doses with dose 4 on or after 12 months of age; or 3 doses with 1 dose on or after 12 months of age; or 2 doses with both doses on or after 12 months of age; or 1 dose on or after 24 months of age; or if the current age is 5 years or older, no new or additional doses are required

Polio Day Care, Head Start, K-12 Day Care, Head Start- 3-4 months: 1 dose due. 5-15 months: 2 doses due. 16 months to K: 3 doses due. 4 doses are required if a combination of IPV and OPV are used.

K-12- 3 doses; K-6: last dose after 4th birthday. 4 doses are required if a combination of IPV and OPV are used.

Varicella

Day Care, Head Start, K-7

Day Care - Head Start - 19 months and older: 1 dose.

K-4th grade - 2 doses.

5th-9th grade - 1 dose.

As satisfactory evidence of disease, an MD or DO may sign and place on file with the school a written statement documenting the month and year of previous varicella (chickenpox) disease.
