Vaccine Requirements for the State of West Virginia

Measles, Mumps, Rubella (MMR) Day Care, K-12 Day Care, K-12- 1 dose measles and rubella after 1st birthday; mumps not required

K- 2nd measles required prior to entry

Pneumococcal None/No Requirements None/No Requirements
Polio Day Care Day Care- Age appropriate

New Entrants- 3 doses, last dose after 4th birthday
