Vaccine Requirements for the State of Idaho
Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Pre-K-12: Children born on or before September 1, 1999 must have a minimum of 4 doses of DTaP. Children born after September 1, 1999 through September 1, 2005 must have a minimum of 5 doses.The 5th dose is not necessary if the 4th dose was administered at age 4 years or older. 7th grade entry - One dose of Tdap. |
| Hepatitis A | K-12 | Children born after September 1, 2005 must have a minimum of two doses. |
| Hepatitis B | Day Care, K-12 | Day Care, K-12- 3 doses. Birth: 1 dose, 2 months: 2nd dose, 6 months: 3rd dose |
