Vaccine Requirements for the State of California
Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

| Diphtheria, Tetanus, Pertussis (DTaP), Tdap | Day Care, Head Start, K-12 | Day Care, Head Start- 1 dose for 2-3 months, 2 doses for 4-5 months, 3 doses for 6-17 months, 4 doses for 18 months - 4 years of age, (Record may show DT or DTP)
K-12 - Ages 6 years and under: 5 doses at any age, but 4 doses meet requirements for ages 4-6 years if at least 1 was on or after the 4th birthday. Ages 7 years and older: 4 doses at any age, but 3 doses meet requirement for ages 7-17 years if at least 1 was on or after the 2nd birthday. If last dose was given before 2nd birthday, one more (Td) dose is required. Pertussis is not required after age 6. 7th grade - 1 dose on or after the 7th birthday. |
