Vaccine Requirements for the State of Washington

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start, K-12 Child Care, Head Start- Ages 3-4 months: 1 dose required. Ages 5-6 months: 2 doses. 7 months to 19 months: 3 doses. 19 months and older: 4 doses.

K - 5th grade: 5 doses of DTaP provided the last dose is given on or after 4th birthday.

Grades 6-12: 5 doses of DTaP and 1 dose of Tdap.

Hepatitis A None/No Requirements Vaccine available statewide for ages 2-18.
Hepatitis B Day Care, Head Start, K-12 Child Care, Head Start, Ages 3-4 months: 1 dose required; Ages 5-19 months: 2 doses required. 19 months and older: 3 doses required.

K-12: 3 doses required

Hib Day Care, Head Start Child Care, Head Start- 3-4 months: 1 dose required. 5-15 months: 2 doses required. One dose is required on or after the 1st birthday if it is the 2nd or 3rd dose.

15 to 59 months: 1 dose required. Only 1 dose is required if it is given on or after 15 months of age.

Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12 Child Care, Head Start,: 1 dose MMR given on or after 1st birthday.

Grades K-2: 2 doses measles; 1 dose mumps and 1 dose rubella both given on or after the 1st birthday and at least 28 days apart.

Grades 3-5: 1 dose MMR provided it was given on or after the 1st birthday.

Grades 6-12: 2 doses measles; 1 dose mumps and 1 dose rubella both given on or after the 1st birthday and at least 28 days apart.

2nd dose - New entry 2 measles after 1st birthday

Meningococcal None/No Requirements None/No Requirements
Pneumococcal Day Care, Head Start Age 3 months: 1 dose; 5 months, 2 doses; 7 months: 3 doses; 16 months: 4 doses.
Polio Day Care, Head Start, K-12 Child Care, Head Start- Age 3-4 months: 1 dose required. Age-5-19 months: 2 doses. 19 months and older: 3 doses.

K-12 - 4 doses provided last dose was given on after 4th birthday

Varicella Day Care, Kindergarten, Sixth Grade

Day Care, Head Start: 1 dose given by 16 months of age but not before 12 months.

K-6: 2 doses or healthcare provider’s proof of disease.
