Vaccine Requirements for the State of South Carolina

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start, K-12, College/University Day Care, Head Start- 4 doses of any combination of DTP, DT, DTP or DTaP, with at least 1 dose received on or after the 4th birthday.

K-10- 4 doses of any combination of DTP, DT, DTaP, Td, or Tdap vaccine with at least one dose received on or after the fourth birthday.

11th & 12th: Three (3) doses of any combination of DTP, DT, DTaP, Td, or Tdap vaccine with at least one (1) dose received on or after the fourth birthday

Hepatitis A None/No Requirements None/No Requirements
Hepatitis B Day Care, Head Start, Kindergarten, First Grade, Second Grade, Third Grade, Fourth Grade, Seventh Grade, Eighth Grade, Ninth Grade, Tenth Grade, College/University Day Care, Head Start- three doses or age appropriate

K-5, 7-11- 3 doses

College/University- as recommended by the ACIP for all enrollees

Hib Day Care, Head Start Day Care, Head Start- Age appropriate. For children 15-59 months of age who have not yet completed age-appropriate HIB vaccination, 1 dose of Hib vaccine at or after 15 months of age is required.
Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12, College/University Day Care, Head Start, K-12- 2 doses of Measles vaccine with both doses received on or after 1st birthday and separated by at least 1 month. 1 dose of Rubella vaccine received on or after 1st birthday. 1st dose of Mumps vaccine received on or after the 1st birthday.

K-12- second dose measles with both doses received on or after 1st birthday and separated by at least 1 month.

Meningococcal None/No Requirements None/No Requirements
Pneumococcal Day Care, Head Start Current, age-appropriate Pneumococcal vaccination according to the currently published immunization schedule. For children aged 24-59 months who have not yet completed any age-appropriate pneumococcal vaccination schedule (PCV7 or PCV13), one (1) dose of 13-valent pneumococcal conjugate vaccine on or after the 2nd birthday is required. Pneumococcal conjugate vaccine is not required for children 5 years of age and older.
Polio Day Care, Head Start, K-12, College/University Day Care, Head Start- 3 doses of any combination of oral or inactivated Polio vaccine with at least 1 dose received on or after the 1st birthday

K-12- 3 doses of any combination of oral or inactivated Polio vaccine with at least 1 dose received on or after the 1st birthday

Varicella Day Care, Head Start, K-12

Day Care: 1 dose. 5K: Two (2) doses of Varicella vaccine with both doses received on or after the first birthday separated by 12 weeks (children <13 years) or 4 weeks (children > 13 years) or positive history of disease.

1 to 12th: One (1) dose of Varicella vaccine with received on or after the first birthday or positive history of disease
