Vaccine Requirements for the State of North Dakota

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start, K-12 Day Care, Head Start-  4 doses, age appropriate.

K-12- 5 doses. One dose of DTaP (pediatric diphtheria, tetanus, and acellular pertussis) vaccine must have been given on or after the 4th birthday. Only four doses are necessary if the 4th dose was administered on or after the 4th birthday. Three doses of Tdap (adolescent/adult tetanus, diphtheria, and acellular pertussis)/Td are required for children ages seven or older who were not previously vaccinated. Tdap should be used as the first dose followed by two doses of Td for children age seven or older not previously vaccinated.

Rotavirus Day Care, Head Start

Children who do not receive the first dose by 15 weeks of age can no longer receive this vaccine and are exempt from the Rotavirus requirement. Children ages eight months and older are exempt from the Rotavirus vaccination requirement.

Rotavirus vaccine may be given as a two or three dose series depending on the brand of vaccine. The third dose of rotavirus vaccine may not be necessary depending on the brand of rotavirus vaccine given. Hepatitis B Kindergarten, First Grade, Second Grade K-2 - 3 doses, age appropriate. No requirement for day care or head start.
Hib Day Care, Head Start Day Care, Head Start - 3 or 4 doses, age appropriate. If a child receives immunizations late, fewer doses may be required. Contact your local public health unit or the North Dakota Department of Health to determine the appropriate number of doses. Children ages five and older are exempt from the Hib requirement. Depending on vaccine brand the child may receive a series of three or four doses
Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12, College/University Day Care, Head Start, K-12 - DC and HS requre 1 dose after the 1st birthday. Grades K-10 require 2 doses. Grades 11-12: 1 dose.

College, University- MMR, 2nd dose measles is required for all enrollees born after 1/1/57

K-7 2nd dose measles is required prior to entry

Hepatitis A Day Care, Head Start 2 doses: 1st dose at 12-17 months; 2nd dose at 18-24 months of age.
Pneumococcal Day Care, Head Start 3 or 4 doses.
If a child receives immunizations late, fewer doses may be required. Contact your local public health unit or the North 
Dakota Department of Health to determine the appropriate number of doses. Children age five and older are exempt 
from the PCV requirement.

Polio Day Care, Head Start, K-12 Day Care, Head Start- 3 doses, age appropriate.

K-12- 4 doses. For polio vaccination, in all-IPV or all-OPV schedule: one dose must have been given on or after the 4th birthday. The final dose in the series should be administered on or after the 4 th birthday and at least six months after the previous dose. If four doses are administered prior to age four a 5 th dose should be administered at age four through 6 years. Only three doses of IPV are required if the 3 rd dose is given on or after the 4th birthday

Varicella Day Care, Kindergarten

Day Care: 1 dose. Children with a reliable history of chickenpox disease are exempt from the varicella requirement

K-6th: For the 2014-15 school year, two doses of varicella vaccine are required for kindergarten through sixth grade. If a child has a reliable history of chickenpox disease, the child is exempt from the vaccine requirement.

7th-12th: For the 2014-15 school year, one dose of varicella vaccine is required of children attending seventh through tenth grade. If a child has a reliable history of chickenpox disease, the child is exempt from the vaccine requirement
