Vaccine Requirements for the State of Montana

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start, K-12, Seventh Grade Day Care, Head Start-  4 doses - Each child must receive at least four doses of DTP or DTaP (diphtheria, tetanus and pertussis) vaccines by age four and one dose of DTaP after age four but before age seven, unless a licensed health care provider has issued a medical exemption for the pertussis portion of the DTP or DTaP vaccine. If a medical exemption has been issued for pertussis, the child must receive at least four doses of DT vaccine or a combination of four doses of DT, DTP, and DTaP vaccines before age four and one dose of the DT vaccine after age four but before age seven.

K-12-  4 doses, 1 dose after 4th birthday;  3 doses Td if started after age 7.

7th grade:  1 dose of vaccine containing Td if the following criteria are met: (i) at least a five year interval must have passed since the previous doses of DTP, DTaP, DT or Td; (ii) the student is 11 years of age or older; or (iii) a dose of Td was not given at seven years of age or older; (d) If a student enters the seventh grade before reaching 11 years of age, a booster shot of Td must be administered as soon as possible after attaining that age, unless the pupil already was administered a dose of Td at seven years of age or older; (e) DT vaccine administered to pupils less than seven years of age is acceptable only if accompanied by a medical exemption that exempts the pupil from pertussis vaccination.

Hepatitis B Day Care, Head Start, K-12 Day Care, Head Start, K-12 - Born on or after 1/1/95: 3 doses
Hib Day Care, Head Start Day Care, Head Start- 2-3 months: 1 dose due. 4-59 months, 2nd dose due. At least one dose must be given after 12 months of age.
Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12, College/University Day Care, Head Start, K-12- 1 dose after 1st birthday, due by 16 months.

2005-2006 school year: Children entering Kindergarten or first grade (for those who did not go to Kindergarten) will need evidence of 2 doses of MMR after 1st birthday.

College, University- MMR, 2nd dose measles required for all enrollees: MMR is required for full time enrollees born after 1957

7-12 - Required at age 13-MMR required prior to entry
