Vaccine Requirements for the State of Michigan

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start Day Care, Head Start- 2-3 months: 1 dose. 4-5 months: 2 doses. 6-14 months: 3 doses. 15 months - 6 years: 4 doses, 1 dose must be on or after 4th birthday.

K-12 - 7-18 years 4 doses D and T or 3 doses Td if #1 given on or after 7 years of age.  

1 dose of Tdap*** for children 11 through 18 
years 
IF
 5 years since the last dose of 
tetanus/diphtheria containing vaccine.
1 dose of Tdap for children 11 through 18 years if 5 years since the last dose of tetanus/diphtheria containing vaccine.

 

Hepatitis A None/No Requirements None/No Requirements
Hepatitis B Day Care, Head Start, K-12 Day Care, Head Start, K-12 - 2-3 months: 1 dose. 4-5 months: 2 doses. 15 months through 18 years: 3 doses. All 6th graders will be assessed for 3 doses.
Hib Day Care, Head Start Day Care, Head Start- 2-3 months: 1 dose due. 4-14 months, 2nd dose due. 1 dose after or on 15th month or completed series earlier.
Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12 Day Care, Head Start, K-12- 1 dose after 1st birthday, 4 years through 18 years: 2 doses on or after 12 months. All 6th graders will be assessed for 2 doses of MMR.

7-12 - MMR required prior to entry

Meningococcal 6th grade 1 dose for children 11 years of age or older upon entry into 6 th grade or higher
Pneumococcal Day Care, Head Start

2-3 months: 1 dose. 
4-5 months: 2 doses.
6-15 months: 3 doses.
16-18 months: 4 doses OR age appropriate complete series.
19 months - 4 years: 1 dose on or after 24 months OR age appropriate complete series.

Polio Day Care, Head Start, K-12 Day Care, Head Start- 2-3 months: 1 dose due. 4-14 months: 2nd dose due. 15 months to 4 years: 3rd dose due.

K-12 - 3 doses, 1 dose must be on or after 4th birthday. 7-18 years: 3 doses. All 6th graders will be assessed for 3 doses of polio.

Varicella Day Care, Head Start, K-12 Day Care, Head Start, K-12 - 15 months - 4 years: 1 dose on or after 12 months. 4 years through 18 years: 1 dose if given on or after 12 months OR 2 doses if initiated on or after 13th birthday. All 6th graders will be assessed for varicella immunity (either with the vaccine or history of disease).
