Doses given at 2 months, 4 months, 6 months, 15-18 months (4th dose may be given at 12 months provided at least 6 months after dose 3) and 5 years of age. Four doses acceptable if dose 4 given on or after the 4th birthday and 6 month after 3rd dose. |
Hepatitis A | Day Care, Head Start | 2 doses required for children less than 5 years of age. The first dose is given at 12 to 18 months of age, with a 6 month interval between the first and second dose. |
Hepatitis B | Head Start, Kindergarten | Head Start, K-12 - 3 three doses required through grade 12. Doses given at birth, 2 months, and 6-18 months of age |
