Vaccine Requirements for the State of Kansas

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start, K-12 Day Care, Head Start - 2 months: 1 DTP/DTaP due. 4 months: 2 doses due. 6 months: 3 doses due. 12-15 months: 4 doses due.

K-12 - 4 doses, 1 dose must be on after 4th birthday; 3 doses. Td booster needed 10 years after completion of primary series.

Hepatitis A None/No Requirements None/No Requirements
Hepatitis B Head Start, Kindergarten Head Start- 3 doses required for all enrolles. Required for children entering Kindergarten for the 2004-2005 school year.
Hib Head Start Day Care - not required

Head Start - 1 dose after 1st birthday

Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12 Day Care, Head Start, K-12- 1st dose on or before the 1st birthday with the 2nd dose given at 4-6 years of age or 4 weeks after the 1st dose.

Pneumococcal None/No Requirements None/No Requirements
Polio Day Care, Head Start, K-12 Day Care, Head Start- 2 months: 1 dose due. 4 months: 2nd dose due. 6 months: 3rd dose due. 12-15 months: 4th dose due.

K-12- 4 doses unless 3rd dose after 4th birthday, then 3 doses

Varicella Kindergarten Required for children entering Kindergarten for the 2004-2005 school year.
