Vaccine Requirements for the State of District of Columbia

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start, K-12, College/University Day Care, Head Start - 4 or 5 doses: 1st dose to be administered no later than 2 months of age. 2nd dose to be administered 2 months after 1st dose. 3rd dose to be administered 2 months after the 2nd dose. 4th dose to be administered 6 months after the 3rd dose. 5th dose (or booster) to be administered at 4 years of age.

K-5th- 5 doses thru 6 years of age unless 4th dose given after 4th birthday; 3 doses after 6 years of age. An additional Td booster dose shall be administered, if more than 10 years have passed since primary series or last booster.

6th-12th grade - 1 Tdap (if five years since last dose of DTP/DTaP/Td)

College, University- All enrolless under 26 years of age

Hepatitis A Day Care, Head Start, Kindergarten 2 doses required
Hepatitis B Day Care, Head Start, Kindergarten, First Grade, Seventh Grade Day Care, Head Start, K-12 - Dose #1 to be administered at birth to one month of age. Dose #2 to be administered at 2 months of age. Dose #3 to be administered no earlier than 2 months after the 2nd dose, and 4 months after dose #1.
Hib Day Care, Head Start Day Care, Head Start- 3 or 4 doses depending on manufacturer.
Human Papilloma Virus Sixth grade 6th - 12th - 3 doses of HPV vaccine required for females in grades 6  thru 9 or parent may sign approved vaccine refusal form  available at www.doh.dc.gov
Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12, College/University Day Care, Head Start, K-12- 1 dose after 15 months of age. 2nd dose to be administered at 4 years of age.

2nd dose- K-1, 7-12- 2nd dose MMR after 4th birthday

College, University- All enrollees under 26 years of age

Meningococcal Sixth 6th -12th - 1 dose required
Pneumococcal Day Care, Head Start 4 PCV required
Polio Day Care, Head Start, K-12 Day Care, Head Start- OPV 4 doses prior to 4th birthday, unless 4th dose after 4th birthday; IPV- 2 doses prior to 12 months of age, 3 doses after 1st birthday unless 3rd dose given after 4th birthday; 5 doses if 4 doses given prior to 4th birthday

K-12- OPV or IPV 3 doses until 18 years of age; IPV-OPV schedule follows IPV schedule only

College, University- if under 19 years of age

Varicella Day Care, Head Start, K-12

Day Care, Head Start - 1 dose if no history of disease.

K-12 - 2 doses are required.
