Vaccine Requirements for the State of Louisiana

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start, K-12 Day Care, Head Start- Age appropriate (means that the number of doses of a vaccine a child should have received is dependent upon the child’s age, and this defined by CDC at www.cdc.gov/nip)

K-12- New Entrants- 4-5 doses, 1 dose after 4th birthday

College, University- Td- Freshman and new enrollees

Hepatitis A None/No Requirements None/No Requirements
Hepatitis B Kindergarten K-12 - 3 doses, age appropriate (means that the number of doses of a vaccine a child should have received is dependent upon the child’s age, and this defined by CDC at www.cdc.gov/nip)
Hib Day Care, Head Start Day Care, Head Start- Age appropriate
Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12 Day Care, Head Start, New Entrants (K-12)- 2 doses, 1 dose after 1st birthday

New Entrants- Required prior to entry

Meningococcal Sixth Grade Beginning with the 2009-2010 school year and continuing thereafter, a student shall provide satisfactory evidence of current immunizations against meningococcal disease, and any other age appropriate vaccines, as a condition of entry into the sixth grade. Further, any student who has attained the age of eleven years or who is entering grade other than grade six shall provide satisfactory evidence of current immunizations against meningococcal disease and any other age appropriate vaccines as a condition of entry into that grade.
Pneumococcal Day Care, Head Start, Kindergarten Pneumococcal Conjugate Vaccine (PCV7 – for children less than 24 months of age);
Polio Day Care, Head Start, K-12 Day Care, Head Start - Age appropriate (means that the number of doses of a vaccine a child should have received is dependent upon the child’s age, and this defined by CDC at www.cdc.gov/nip)

K-12 - 4 doses, last dose after 4th birthday

Varicella Day Care, Kindergarten, Sixth Grade, Pre-K Beginning in school year 2009-2010, two (2) doses of Varicella vaccine shall be required in Louisiana schools for entry into Pre-K, Kindergarten, Daycare, and HeadStart programs for children aged 4 years and older. If a second dose of Varicella vaccine has been received at least 30 days after the first dose, no additional doses are required. This is in addition to the regular age appropriate vaccines required depending on the child’s age.
