Vaccine Requirements for the State of Illinois

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start, K-12, College/University Day Care, Head Start - Any child 2 years of age or older must have 4 doses.

K-12- 4 doses is only required of those children entering kindergarten (or 1st grade if the school does not have a kindergarten class). 3 doses for children entering at any other grade level, with the last dose received on or after the 4th birthday.

College, University- 1 dose for all students, except international students who must provide proof of 3 doses.

Hepatitis A None/No Requirements None/No Requirements
Hepatitis B Day Care, Head Start, K-12 Day Care, Head Start, Grade 5-10 - 3 doses for students who have entered the 5th grade since
Fall 1997.
Hib Day Care, Head Start Day Care, Head Start- 2-6 months: 2 or 3 doses (depending on vaccine used), 2 months apart. 7-11 months: 2 doses, 2 months apart. 12-14 months: 1 dose. 15-59 months: 1 dose
Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12, College/University Day Care Head Start - Day Care/Head Start - 1 dose on or after the 1st birthday.

Kindergarten, 1, 2 - 1st dose must be received on or after 1st birthday or positive history of disease.

K-12- 2 doses of measles, 1 dose of mumps and 1 dose of rubella. Interval between 1st and 2nd dose no less than 4 weeks.

2nd dose - Children born on or after September 2, 1991: two doses of measles vaccine administered on or after the first birthday and at least 28 days apart. Children born prior to September 2, 1991 will be required to show proof by 30 days past their 12th birthday of either: two doses of measles vaccine administered on or after the first birthday and at least 28 days apart.

College, University- MMR, 2nd dose measles-All freshman and new enrollees in 4-year schools born after 1956

Polio Day Care, Head Start, K-12 Day Care: 2 doses by age 1 year; over age 2: 3 doses; Head Start- 3 doses;

K-12- 3 doses if all-IPV or all-OPV received. 4 doses if any combination of IPV & OPV. In all cases, the last dose must be received on or after the 4th birthday.

Varicella Day Care, Head Start, Kindergarten Day Care, Head Start, Kindergarten - 1 dose for those children entering a day care center, Head Start center and kindergarten for the 1st time on or after 7/1/02. Disease history, if verified by a health care provider, is also acceptable.
