Vaccine Requirements for the State of Alabama

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start, K-12 Day Care, Head Start - Day Care, Head Start – 4 doses or age appropriate. (Age appropriate means that the number of doses of a vaccine a child should have received is dependent upon the child’s age, and this defined by CDC at www.cdc.gov)

K-12 - 5 doses of diphtheria and tetanus toxoids and acellular pertussis vaccine (DTaP). Only 4 doses are needed if the fourth dose was administered on or after the fourth birthday. Booster doses of tetanus and diphtheria toxoids vaccine (Td) must be given 5-10 years after the preschool booster. However, effective for students entering sixth grade beginning fall of 2010, a booster dose of tetanus and diphtheria toxoids and acellular pertussis vaccine (Tdap) must be given at 11 or 12 years of age. This requirement will escalate by one successive grade each year for the following 6 years to include sixth through twelfth grades, beginning fall of 2016.

Hib Day Care, Head Start Day Care, Head Start - Age appropriate (means that the number of doses of a vaccine a child should have received is dependent upon the child’s age, and this defined by CDC at www.cdc.gov)
Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12, College/University Day care, Head Start: 1 doses required. The first dose must have been administered on or after the child’s 1st birthday. 1 dose of rubella and 1 dose of mumps are required, each administered on or after the 1st birthday.

K-12: 1 dose of rubella and 1 dose of mumps are required, each administered on or after the 1st birthday.

Second dose - 2 MCV: 1st dose on/after 1st birthday and 2nd dose after 28 days

College/University - 2 doses MMR required for all entrants to four year colleges.

K-12 - 4 doses (3 if the 3rd dose was received after the fourth birthday).

Varicella Day Care, Head Start, Kindergarten, First Grade, Second Grade, Third Grade, Fourth Grade

Varicella vaccine subject to the following schedule unless there is documentation of a positive varicella titer or a date of varicella disease. This requirement is effective for students entering kindergarten beginning fall of 2001 and will escalate by one successive grade each year for the following 12 years to include all grades, kindergarten through twelfth, beginning fall of 2013:


(i) 1 dose of varicella vaccine at 12 months of age or older for persons less than 13 years of age;

(ii) 2 doses of varicella vaccine separated by at least 28 days for persons 13 years of age or older beginning the vaccination series.
