Vaccine Requirements for the State of Vermont

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Head Start, K-12, College/University Day Care, Head Start-  Age appropriate (means that the number of doses of a vaccine a child should have received is dependent upon the child’s age, and this defined by CDC at www.cdc.gov/nip)   up to 3 doses, the last 2 doses should be separated by at least 6 months. Pertussis not required. 

K-12-  3 doses with 6 months between 2nd and 3rd dose;  10 year booster dose of Td; pertussis not required.

Effective August, 2008:  K- 5 DTaP – 4 if the 4th dose was given on/after the 4th birthday. 
7th grade- 5 DTaP/DTP – 4 doses if the 4th dose was given on/after the 4th birthday. For pupils who, after the sixth birthday, have not completed these requirements three doses of Td or 2 doses of Td and one dose of Tdap is acceptable.
Post-secondary school- 1 Tdap/Td booster administered within the past 10 years.

Hepatitis B Seventh Grade Grade 7-  effective 8/99

Effective August, 2008: K-12, Post secondary- 3 doses.

Hib Day Care, Head Start Day Care, Head Start- Age appropriate
Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12, College/University Day Care, Head Start, K-12-  K-12: 1 dose MR after 1st birthday, mumps not required

 K-12-  required prior to entry

College, University-  All new enrollees; M2: all foreign, cosmetology, and health science students

Effective August, 2008: K-12, Post-secondary- 2 doses.

Meningococcal Seventh Grade, College/University Effective August, 2008: 7th grade- 1 dose required for all students living in dormitories or campus-based housing.
Post secondary- 1 dose for first year students living in dormitories or campus-based housing.
Pneumococcal None/No Requirements None/No Requirements
Polio Day Care, Head Start, K-12 Day Care, Head Start-  Age appropriate  (means that the number of doses of a vaccine a child should have received is dependent upon the child’s age, and this defined by CDC at www.cdc.gov/nip).

K-12-  Minimum of 3 doses with 6 months between 2nd and subsequent dose. 

Effective August, 2008: K-12- 4 doses are required unless the third dose of an all-IPV or all-OPV schedule is given on or after the 4th birthday, in which case only 3 doses are required. However, if the sequential or a mixed IPV/OPV schedule was used, 4 doses are always required to complete the primary series.

Varicella Kindergarten, Seventh Grade, College/University Effective August, 2008: 2 doses. The requirement is waived if parent/guardian signs a Department-supplied form indicating the student has a history of disease.
