Vaccine Requirements for the State of Vermont
Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

| Diphtheria, Tetanus, Pertussis (DTaP) | Head Start, K-12, College/University | Day Care, Head Start- Age appropriate (means that the number of doses of a vaccine a child should have received is dependent upon the child’s age, and this defined by CDC at www.cdc.gov/nip) up to 3 doses, the last 2 doses should be separated by at least 6 months. Pertussis not required.
K-12- 3 doses with 6 months between 2nd and 3rd dose; 10 year booster dose of Td; pertussis not required. Effective August, 2008: K- 5 DTaP – 4 if the 4th dose was given on/after the 4th birthday.

K-12- Minimum of 3 doses with 6 months between 2nd and subsequent dose. Effective August, 2008: K-12- 4 doses are required unless the third dose of an all-IPV or all-OPV schedule is given on or after the 4th birthday, in which case only 3 doses are required. However, if the sequential or a mixed IPV/OPV schedule was used, 4 doses are always required to complete the primary series. |
| Varicella | Kindergarten, Seventh Grade, College/University | Effective August, 2008: 2 doses. The requirement is waived if parent/guardian signs a Department-supplied form indicating the student has a history of disease. |
