Vaccine Requirements for the State of Tennessee

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start, K-12 Day Care, Head Start - 4 or more doses, age appropriate.

K-12 - 4 or more doses, age appropriate.

7th grade: Tdap booster - evidence of one Tdap dose given any time before 7th grade entry is required regardless of Td history.

Hepatitis A Day Care, Head Start, K Day Care, Head Start: 1 dose, required by 18 months of age or older. K: 2 doses, spaced at least 6 months apart
Hepatitis B Day Care, Head Start, Kindergarten Day Care, Head Start, Kindergarten - 3 doses, age appropriate.
Hib Day Care, Head Start Day Care, Head Start- Age appropriate, only for ages 5 and younger.
Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12, College/University Day Care, Head Start, K-12 - 2 doses, given on or after the 1st birthday required.

College/University- two doses of MMR are required for full time students born after 1957

K-12- second dose of MMR is required prior to entry

Meningococcal None/No Requirements None/No Requirements
Pneumococcal Day Care, Head Start Day Care, Head Start- age appropriate, kids 5 and younger.
Polio Day Care, Head Start, K-12 Day Care, Head Start, K-12 - 4 doses, age appropriate.  Final dose on or after the 4th birthday.
Varicella Day Care, Head Start, K-12 Day Care, Head Start: 1 dose. K, 7th grade: 2 doses or history of disease.
