Vaccine Requirements for the State of New Jersey

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start, K-12, Sixth Grade Day Care, Head Start- 4 doses or age appropriate (means that the number of doses of a vaccine a child should have received is dependent upon the child’s age, and this defined by CDC at www.cdc.gov/nip) ; one dose must be given on or after the 4th birthday.

K-12- 4 doses, 1 dose after 4th birthday; 3 doses Td for age 7 or older

6th - 1 dose of Tdap. For pupils entering Grade 6 on or after 9-1-08 and born on or after 1-1-97. A child does not need a Tdap dose until FIVE years after the last DTP/DtaP or Td dose.

Hepatitis A None/No Requirements None/No Requirements
Hepatitis B None/No Requirements K, 1, 6 - Every child born on or after 1/1/96 shall have received 3 doses prior to school entry.
Hib Day Care, Head Start Day Care, Head Start- < 1 year: 2 doses; or at least 1 dose after 12 months
Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12 Day Care, Head Start, K-12- 2 doses of Measles vaccine with both doses received on or after the 1st birthday and separated by at least 1 month. 1 dose of Rubella vaccine received on or after the 1st birthday. 1 dose of Mumps vaccine received on or after the first birthday.

College, University- MMR, 2nd dose measles New enrollees of 4 year colleges and 2 year colleges (with 12 credit hours) born after 1956

K-1,2nd dose measles is required prior to entry

Meningococcal Sixth Grade, College/University 6th grade- 1 dose. For pupils entering Grade 6 on or after 9-1-08 and born on or after 1-1-97.

College/university- New students residing in a campus dormitory, shall receive a meningococcal vaccination as a condition of attendance.

Pneumococcal None/No Requirements None/No Requirements
Polio Day Care, Head Start, K-12 Day Care, Head Start- 3 doses; age appropriate (means that the number of doses of a vaccine a child should have received is dependent upon the child’s age, and this defined by CDC at www.cdc.gov/nip) ; one dose must be given after the 4th birthday.

K-12- 3 doses, last dose after 4th birthday

Varicella Day Care, Kindergarten, First Grade Day care, K and First grade - 1 dose of varicella vaccine should be administered on or after 1st birthday.
Every child born on or after January 1, 1998, should have received 1 dose of varicella vaccine before entering school (effective 2004-2005 school year).
