Vaccine Requirements for the State of Nevada
Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

| Diphtheria, Tetanus, Pertussis (DTaP) | Day Care, Head Start, K-12, College/University | Day Care, Head Start- 4 or 5 (If dose #4 is given on or after 4th birthday #5 is not needed), age appropriate, (means that the number of doses of a vaccine a child should have received is dependent upon the child’s age, and this defined by the CDC)
K-12- 4 doses, 1 dose after 4th birthday 7th grade: 1 dose of Tdap. |
| Hepatitis A | K-12 | K-12 - 2 doses at least 6 months apart. |
| Hepatitis B | Day Care, Head Start, College/University | K-12 - 3 doses
College, University- Medical and Nursing students: required before clinical practice |
