Vaccine Requirements for the State of Iowa

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, K-12 Day Care- 2-18 months of age: 1 dose required. 18 months of age and older: 3 doses.

K-12- 3 doses, 1 dose after 4th birthday

Hepatitis A None/No Requirements None/No Requirements
Hepatitis B K-12 3 doses for all children born after 7/1/94
Hib Day Care, Head Start Day Care, Head Start- 2-18 months at least 1 dose required; 18 months or older 3 doses required; or 1 dose after 15 months of age
Measles, Mumps, Rubella (MMR) Day Care, K-12 K-12 - 1 dose measles and rubella required for children 15 months and older; 2nd dose required if enrolled after 7/3/91; mumps not required

Day Care - 1 dose required for children 18 months and older

Polio Day Care, K-12 Day Care- 2-18 months of age: 1 dose required. 18 months of age and older: 3 doses.

K-12- 3 doses, last dose after 4th birthday

Varicella Day Care, K-12 All children born after 9/15/97 and entering a licensed childcare facility, pre-school, or school must receive before entry.
