Vaccine Requirements for the State of Alaska

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start, K-12 Day Care, Head Start - Dose #1 by 3 months of age. Dose #2 should be administered 3 months after dose 1. Dose #3, 3 months after dose 2. Dose #4, one year after dose #3. An additional dose is required if child is in grade K or higher and received last dose before 4 years of age

K-12- 4 doses; 5 doses if 4th dose before 4th birthday; 10 year Td booster required within 10 years of last diphtheria/tetanus.

Hepatitis A Day Care, Head Start, K-12 Day Care, Head Start, K-12 - 2 appropriately spaced doses received on or after 2nd birthday
Hepatitis B Day Care, Head Start, K-12 Day Care, Head Start, K-12 - 3 appropriately spaced doses
Hib Day Care Day Care, Head Start - 3 appropriately spaced doses. Under 5 years only.
Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12 Day Care, Head Start, K-12 - 1 dose on or after 1st birthday

2nd dose of measles - Required only for children in grades K-12.

Polio Day Care, K-12 Day Care - 3 appropriately spaced doses or age appropriate (means that the number of doses of a vaccine a child should have received is dependent upon the child’s age, and this is defined by CDC at www.cdc.gov).

K-12 - <6 years of age: 3 doses, last dose after 4th birthday; >6 years of age: 3 doses

Varicella Day Care, Head Start

Day Care, Head Start - 1 dose on or after 1st birthday

K-6 - 2 doses
