Vaccine Requirements for the State of Idaho

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start, K-12

Pre-K-12:  Children born on or before September 1, 1999 must have a minimum of 4 doses of DTaP. Children born after September 1, 1999 through September 1, 2005 must have a minimum of 5 doses.The 5th dose is not necessary if the 4th dose was administered at age 4 years or older.

7th grade entry - One dose of Tdap.

Hepatitis A K-12 Children born after September 1, 2005 must have a minimum of two doses.
Hepatitis B Day Care, K-12 Day Care, K-12- 3 doses. Birth: 1 dose, 2 months: 2nd dose, 6 months: 3rd dose
Hib Day Care Day Care- 2 months: 1st dose, 4 months: 2nd dose, 6 months: 3rd dose, 12-18 months: 4th dose
Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12 Day Care, Head Start: 1-4 years, 1 dose. 4-6 years, 2 doses.

Pre-K-12: 2 doses. Both doses must be administered on or after the 1st birthday

Meningococcal 7th grade Children admitted to 7th grade must have one dose of meningococcal vaccine.
Pneumococcal Pre-school 2 months: 1st dose due. 4 months: 2nd dose due. 6 months: 3rd dose due. 12-18 months: 4th dose due.
Polio Day Care, Head Start, K-12 Day Care, Head Start- 2 months: 1st dose due. 4 months: 2nd dose due. 6 months: 3rd dose due only if OPV (oral) used for first 2 doses. 12-18 months: 3rd dose due if IPV (injectable) used for first 2 doses. 4-6 years: 4th dose due.

Pre-K-12: 3 doses required. A 4th dose is recommended for all school age children.

Varicella K-12 Children born after September 1, 2005 must have a minimum of 2 doses. History of varicella (chickenpox) disease documented by a physician or licensed health care professional meets the requirement.
