Vaccine Requirements for the State of Arizona

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start, K-12 Day Care, Head Start- 2-3 months of age: 1 dose DTaP or DT or DTP. 4-5 months of age: 2 doses Dtap or DT or DTP. 6-11 months of age: 3 doses DTaP or DT or DTP. 12 months to 4 years of age: 4 doses Dtap or DT or DTP.

K-12- 4 doses; 5 doses if 4th dose before 4th birthday of DTaP or DT or DTP. 7th-8th grade: td booster recommended if more than 5 years since last DTaP/DTP/DT. 9th-12th grade: 1 Td booster if 10 years since last DTaP/DTP/DT.

Hepatitis A Day Care, Head Start Day Care, Head Start-  Children 2-5 years of age attending licensed or certified DC or HS in Maricopa County only.
Hepatitis B Day Care, Head Start, K-12 Day Care, Head Start:-  Less than 2 months of age:  1 dose required.   2-5 months of age: 2 doses required. 

K-12: 3 doses required. A 2-dose adolescent vaccine may be given to students 11-15 years of age over a 4-6 month period.

Hib Day Care, Head Start Day Care, Head Start- 2 to 3 months of age: 1 dose required. 4-5 months: 2 doses required. 6-11 months: 3 doses required. 12 to 18 months 3 or 4 doses required. (1 must be given after 1st birthday)
Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12 K-12:  2 doses required. Both must be on or after the 1st birthday.
Meningococcal None/No Requirements None/No Requirements
Pneumococcal None/No Requirements None/No Requirements
Polio Day Care, Head Start, K-12 Day Care, Head Start- 2-3 months of age: 1 dose required. 4 to 5 months of age: 2 doses required. 6 months to 4 years of age: 3 doses required. K-12: 3 or 4 doses (only 3 if one was given after 4th birthday).

K-12- 4 doses unless 3rd dose after 4th birthday, then 3 doses

Varicella Day Care, Head Start, Kindergarten, First Grade, Seventh Grade Proof of immunization or history of varicella infection.
