Day Care, Head Start - Day Care, Head Start � 4 doses or age appropriate. (Age appropriate means that the number of doses of a vaccine a child should have received is dependent upon the child's age, and this defined by CDC at www.cdc.gov/nip)

K-12 - 3 doses; at least one dose must have been administered after the child's 4th birthday. Pertussis required.

Hib Day Care;Head Start

Measles, Mumps, Rubella (MMR) Day Care;Head Start;K-12;College/University

Day care, Head Start: 1 doses� required.� The first dose must have been administered on or after the child's 1st birthday. 1 dose of rubella and 1 dose of mumps are required, each administered on or after the 1st birthday.�

K-12: 1 dose of rubella and 1 dose of mumps are required, each administered on or after the 1st birthday.

Second dose - 2 MCV: 1st dose on/after 1st birthday and 2nd dose after 28 days

College/University - 2 doses MMR required for all entrants to four year colleges.

Meningococcal None/No Requirements None/No Requirements

Polio Day Care;Head Start;K-12

K-12�- 4 doses (3 if the 3rd dose was received after the fourth birthday).

Varicella Day Care;Head Start;Kindergarten;First Grade

Day Care, Head Start, K-1�- DC, HS: Age appropriate (means that the number of doses of a vaccine a child should have received is dependent upon the child's age, and this defined by CDC at www.cdc.gov/nip).� Attendees of 5-year old K and 1st grade must have documentation of varicella vaccine.�Documented history of the disease is accepted in lieu of vaccination.
