Vaccine Requirements for the State of Arizona

Vaccine requirements may be relaxed or eliminated in some regions and/or states due to shortages in the manufacture of some vaccines. Please contact your healthcare provider, or your local public health department for more information.

Vaccine Age/Grade Comments
Diphtheria, Tetanus, Pertussis (DTaP) Day Care, Head Start, K-12 Day Care, Head Start- 4-5 doses. At least 1 dose at 4 years of age or older is required. A 6th dose is required if 5 doses have been given before 4 years of age.

K-5th grade- Hostory of 4 DTaP pr a total of 3 tetanus & diphtheria doses given after 12 months of age.

6th-11th grade: 1 Tdap dose is required when 5 years have passed since their last DTaP. DTP, DT or Td. Students starting or finishing the first 3 tetanus & diphtheria doses of their lifetime must receive only 1 Tdap as part of the 3-dose series.

12th - Students who have not already received Tdap are required to receive 1 Tdap dose when 10 years have passed since their last DTaP, DTP, Dt or Td.

Hepatitis A Day Care, Head Start Day Care, Head Start-  Children 2-5 years of age attending licensed or certified DC or HS in Maricopa County only.
Hepatitis B Day Care, Head Start, K-12 Day Care, Head Start, K-12: 3 doses required. A 4th dose will be required if the third dose was given before 24 weeks of age.
Hib Day Care, Head Start Day Care, Head Start- 2 to 3 months of age: 1 dose required. 4-5 months: 2 doses required. 6-11 months: 3 doses required. 12 to 18 months 3 or 4 doses required. (1 must be given after 1st birthday)
Measles, Mumps, Rubella (MMR) Day Care, Head Start, K-12 K-12:  2 doses required. Both must be on or after the 1st birthday. A 3rd dose will be required if dose #1 was given more than 4 days before the 1st birthday.
Meningococcal 6th - 12th

6th-11th - 1 dose.

12th - 1 dose recommended but not required for 12th graders in the 2013-2014 school year.

Pneumococcal None/No Requirements None/No Requirements
Polio Day Care, Head Start, K-12 Day Care, Head Start- 2-3 months of age: 1 dose required. 4 to 5 months of age: 2 doses required. 6 months to 4 years of age: 3 doses required. K-12: 3 or 4 doses (only 3 if one was given after 4th birthday).

K-12- 4 doses unless 3rd dose after 4th birthday, then 3 doses

Varicella Day Care, Head Start, K-12

1 dose is requires if the 1st dose was given before 13 years of age.

2 doses are required if the 1st dose was given at 13 years of age or later.
