The National Network for Immunization Information (NNii) is committed to providing a safe, secure and up-to-date website for everyone who needs to know about immunization issues. It is our policy to create content that requires a minimum collection of information from our visitors, and we employ special techniques and measures to help visitors maintain and protect their personal privacy.

I. The Information We Collect

Site Feedback Form
On each page is a link to an area where visitors can provide feedback on NNii’s website. The visitor is asked to select an audience category that most closely matches their background, several questions assessing their ease and comfort level with the site, and whether or not they plan to visit the site again. The visitor is then asked to provide additional comments if they so desire. Although a visitor may include personally identifiable information on the form, NNii does not require that a visitor give any identifying information in order to provide feedback on its website. If a user chooses to include contact information, this information will not be shared with individuals or organizations outside of NNii.

Resource Kit Feedback Form
On the Healthcare Professionals page is an area where users can provide feedback on “Communicating with Patients about Immunization,” NNii’s resource kit for healthcare professionals. In order to process the user’s request, the following information is required: first name, last name, email address, mailing address, and phone. This information is kept in an NNii database and will not be shared or sold to any individual or organization.

Email Updates to the Resource Kit and Changes of Address for Email Updates to the Kit
On the Healthcare Professionals page is an area where users can request to have email updates to the resource kit sent to them or change any information previously submitted to NNii. In order to process the user’s request, the following information is required: first name, last name, email address, mailing address, and phone. This information is kept in an NNii database and will not be shared or sold to any individual or organization.

Aggregate Log File Data
Log data is information automatically collected from the computers of site visitors. It can be viewed only in aggregate and does not include any information that is personally identifiable. Log data includes such things as the type of computer operating systems, the types of Internet browsers, and the name of the Internet Service Provider used by visitors to the site. Log data also includes quantitative information on which site pages and activities are visited most often. This information aids future site development by showing which features and information users access most.

II. How We Collect the Information

Aggregate log data is collected automatically. “Cookies” are also put into users’ browsers.

III. How We Use the Information

Site and Resource Kit Feedback
Site and Resource Kit Feedback forms are used only to give NNii an idea of who is using the Web site and the resource kit and to collect users’ comments to improve the information offerings.

Email Updates to the Resource Kit and Changes of Address for Email Updates to the Kit
This information is used to fulfill visitors’ requests to receive email updates to the kit and is stored in an NNii database for that purpose.

Use of Non-Personally Identifiable Information (Aggregate Log Data)
Aggregate log data is used to research general site characteristics including a visitor’s browser type and Internet Service Provider.

Security
NNii’s Web site, immunizationinfo.org, maintains an information environment that protects the security, privacy and integrity of all information collected and used within the site. Information is stored within server facilities that use the latest industry standards and practices for protecting information.
